This promotion is intended for participation in the 50 United States and the District of Columbia only and shall be construed and evaluated according to the laws of the United States. Please do not enter this sweepstakes unless you are a legal resident of the 50 United States or the District of Columbia 18 years of age or older and possess a valid CDL.

The Speedco Customer Opinion Sweepstakes

OFFICIAL RULES

  1. TO ENTER: NO PURCHASE NECESSARY TO ENTER OR WIN. Purchase will not increase your chances of winning. Enter the Speedco Fuel for Thought Survey (the "Sweepstakes") sponsored by Speedco, Inc. (the “Sponsor”)by calling from a touch-tone telephone the toll-free number listed on invitations given to customers at participating Speedco stores (the "Participating Stores"), or by logging on to www.opinionport.com/speedco to participate in the survey. You will be asked to register your opinions of a recent visit to a Participating Store and will be automatically entered in the Sweepstakes after doing so. Alternatively, enter by mail by sending a 3X5 card with your name, full address and daytime telephone number to: SRBI, Inc. attn: Speedco Support Team, 400 Olympic Corporate Center, 3940 Olympic Boulevard Erlanger, KY 41018. Limit one telephone, Internet or mail entry every six weeks; all telephone and Internet entries must express opinions of a visit to one of the Participating Stores. Mail entries may express opinions of a visit to one of the Participating Stores but are not required to do so. In the event of a dispute regarding the identity of the person submitting the entry, entry will be deemed submitted by the person in whose name the telephone or Internet account is registered, or whose name and address are listed on the mailed entry, provided that person meets all eligibility criteria.
  2. SWEEPSTAKES PERIOD: The Sweepstakes begins on April 9, 2007 and ends on December 31, 2008. All eligible entries must be received by the end of the Sweepstakes Period.
  3. ELIGIBILITY: The Sweepstakes is open only to legal residents of 50 United States and the District of Columbia, 18 years or older. Employees of Speedco, Inc. or its respective subsidiaries, affiliates, agents, directors, officers, and/or advertising, promotional, or fulfillment agencies of or contracted by any of the foregoing individuals or entities ("Sweepstakes Entities"), and immediate family members and/or persons living in the same household of such employees are not eligible.
  4. WINNER SELECTION: When your entry is received, you have not yet won. Each month during the Sweepstakes Period, one winner will be selected randomly from all eligible entries received by the end of that month. The award process will be conducted by Sponsor or its designee, the judges of the Sweepstakes, whose decisions shall be final and binding on all matters relating to the Sweepstakes. All entries have the same chance of winning. Odds of winning depend upon the total number of eligible entries received. No one will know who the winner is until after the monthly random selection.
  5. PRIZES: PRIZES: Winners will receive a $500 Visa ® Gift Card. Average Retail Value of each prize: $500.00. Estimated Retail Value of all prizes: [$500 X NO. OF MONTHS OF SWEEPSTAKES]. The prizes will be awarded, provided they are properly claimed.
  6. WINNER NOTIFICATION: Winners will be notified by mail and will be required to sign and return an affidavit of eligibility and liability/publicity release within thirty (30) days of notification. If Sponsor is unable to contact a winner despite Sponsor's reasonable efforts to do so, an alternate winner will be selected. Failure to return the signed affidavit of eligibility and liability/publicity release within thirty (30) days of notification, or return of any prize notification or prize as undeliverable, may result in forfeiture of prize and selection of alternate winner, in Sponsor's sole discretion.
  7. GENERAL CONDITIONS: This Sweepstakes is void outside of the United States and the District of Columbia and wherever prohibited by law. All federal, state, and local laws and regulations apply to this Sweepstakes. No prize transfers, cash redemption or substitutions permitted except in the event of unavailability of prize, in which case Sponsor may, in its own discretion, substitute a prize of equal or greater value. Federal law requires that a Form 1099 be supplied to the winner. All federal, state and local taxes on prize, if any, are the sole responsibility of the winner. The Sweepstakes is also subject to these Official Rules. If you cannot or will not comply with these Official Rules, you should not enter or participate in the Sweepstakes. By entering this Sweepstakes, Entrants agree to abide by and be bound by these Official Rules and to waive any right to claim any ambiguity or error in these Official Rules or the Sweepstakes itself. Entrants agree to be bound by all decisions of the Sponsor, whose decisions are final and binding. All entries in the Sweepstakes shall become the sole property of Sponsor and will not be returned to Entrant for any reason. Submission of entry constitutes permission for Sponsor to use the winner's Sweepstakes entry, name, city and state of residence, picture, likeness and biographical information, participation in the Sweepstakes and any material based thereon or derived therefrom, or to refrain from so doing, for all advertising, trade, or promotional uses, without further compensation, worldwide and in perpetuity, in all forms of media, now known or hereafter devised, except where prohibited by law.
  8. PRIVACY POLICY: Sponsor will securely store and will consider confidential all information submitted by Entrants and will not sell or otherwise disclose it to any outside company, except that Sponsor may use Entries and the information submitted by Entrants to help evaluate and improve the operations of stores operated by the Sweepstakes Entities, for the other purposes described in the program, these rules and the affidavit to be signed by Winners, and in the aggregate to track usage patterns and customer base.
  9. DISCLAIMERS: By entering the Sweepstakes, each Entrant releases the Sweepstakes Entities and agents from all responsibility or liability for damages, losses or injuries of any kind resulting from participation in the Sweepstakes, or any prize-related activities, or awarding, possession, use and/or misuse of prize awarded herein. Entrant agrees that Sweepstakes Entities will not be responsible or liable for any entries which are not in compliance with these Official Rules, or for any late, lost, garbled, illegible, damaged, stolen, incomplete, or misdirected entries or data transmission. Sweepstakes Entities are not responsible for technical, hardware, software or telephone malfunctions of any kind, or failed, incorrect, incomplete, inaccurate, garbled, or delayed electronic communications caused by the user or by any of the telephone equipment or computer programming associated with or utilized in this Sweepstakes, or by the failure of any mail or delivery service or any human error which may occur in the processing of the entries in this Sweepstakes. Sponsor reserves the right, in its sole discretion, to cancel, terminate, modify, or suspend the Sweepstakes if the Sweepstakes is not capable of running as planned, due to infection by computer virus, tampering, unauthorized intervention, fraud, technical failures, or any other problems beyond the control of Sponsor. Sponsor reserves the right, at its sole discretion, to disqualify any individual it determines to be tampering with any entry mechanism, to be violating the Official Rules, or acting in a disruptive manner.
